Business Applications & Forms

Find the business applications and forms you need to become a business member with BECU. You'll also find additional forms to manage your existing accounts, apply for loans, or request other financial services. Just fill out, print, and bring the applicable form, plus identification documents, to your nearest BECU Neighborhood Financial Center.

Print and sign (PDF): You can easily download and complete the PDF forms linked below in Acrobat Reader. We recommend downloading forms to your computer before entering any information. You may find PDF forms do not function as expected when opened and completed in a browser like Chrome or Edge.

In order to establish business membership, you will need to open your business account at your nearest BECU location.

In addition to your completed membership application, you will need to bring personal identification (driver's license, passport or government issued ID), your business tax ID (EIN or Social Security number), UBI number (refer to your business license), and NAICS code with you. For corporations, LLCs and partnerships, you will also need to complete the Certification of Beneficial Owner for Legal Entity Members form.

In order to establish business membership, you will need to open your business account at your nearest BECU location.

In order to obtain a business credit card, you can either apply online or at your nearest BECU location.

In order to secure a business term or vehicle loan, you will need to apply at your nearest BECU location.
